Bella Clinical Care offers a Sliding Fee Discount Program to ensure all patients have access to quality healthcare services, regardless of ability to pay.
Discounts are available based on your family size and income.
Patients with income at or below 200% of the Federal Poverty Guidelines may qualify.
To apply, please ask our front desk staff for an application. You will need to provide proof of income to determine eligibility.
No patient will be denied services due to inability to pay.

We offer transparent flat-rate pricing. A $125 deposit is required at check-in for self-pay patients.

By receiving services at Bella Clinical Care, you agree to:
Pay co-pays and deductibles at the time of service
Be responsible for any charges not covered by your insurance
Allow Bella Clinical Care to bill your insurance directly
Labs and other external services may be billed separately.
We do not offer 24/7 on-call services.
For urgent medical concerns outside of regular hours:
Visit your nearest urgent care or emergency room
In a medical emergency, call 911 immediately
If your provider recommends a specialist, our team will help coordinate the referral.
Note: Some insurance plans may require prior authorization, which can take up to 3 business days.
Prescription refills for existing patients are processed during normal business hours.
No refills are issued on weekends, holidays, or after hours
Please call ahead to avoid gaps in medication
To request your records, call our office during business hours.
Requests are not processed on weekends, evenings, or holidays.
Please arrive on time for your scheduled appointment
Late arrivals may be rescheduled based on provider availability
No-call, no-show appointments will be charged a $25 fee
If you need to cancel, please call at least 24 hours in advance
