Insurance / Payment / Billing

At Bella Clinical Care, we accept a wide range of insurances, including BlueCross BlueShield, Medicaid, and Medicare, making it easy for you to access the care you need. At Bella Clinical Care, your health is our top priority. Please contact your insurance company or you may call the location nearest you to find out if we accept your plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.

If you have a question related to your bill or insurance, please ask to speak with a billing specialist when you call.

PATIENT FINANCIAL OBLIGATION AGREEMENT

Thank you for choosing Bella Clinical Care.

We are committed to providing you with the highest quality medical care. The following information is provided to establish clear understanding and agreement concerning payment for our professional services.

I understand that all applicable copayments and deductibles are due at the time of service.

I agree to be financially responsible and make full payment for all charges not covered by my insurance company. I authorize my insurance benefits be paid directly to Bella Clinical Care for services rendered.

I authorize representatives of Bella Clinical Care to release pertinent medical information to my insurance company when requested or to facilitate payment of a claim.

Self-pay patients (do not have insurance):

The balance for your visit will be posted to your statement.

Lab and Radiology services will be billed separately.

After Hours Urgent Medical Needs

At Bella Clinical Care, we prioritize your health and well-being. While we do not offer a 24-hour on-call doctor service, we encourage you to contact your nearest urgent care facility or emergency room for any pressing medical concerns after regular clinic hours. For non-emergency matters, feel free to leave a message, and we will respond promptly during the next business day.

If you or someone you know is experiencing a medical emergency, such as major head trauma, chest pain, severe abdominal pain, or loss of consciousness, you should go directly to the ER or call 911 immediately.

Medical Emergencies

If you are experiencing a life-threatening medical emergency, please call 911 immediately for assistance. Do not delay! Once you arrive at the emergency room, be sure to inform the staff that you are a patient of Bella Clinical Care, and they will reach out to us directly.

Referrals

If one of our primary care provider recommends a specialist consultation or diagnostic test, we will work diligently to arrange your referral promptly. Please note that certain referrals may require prior authorization from your insurance, which can take time to process.

For routine referrals, we ask for up to 3 business days to complete the necessary steps, as we are unable to retroactively authorize referrals due to insurance policies.

Prescription Requests

Prescription requests and refills for established patients of Bella Clinical Care are processed during our regular office hours. To request a refill, please contact our office directly.

Refills cannot be issued during evenings, weekends, or holidays, so we encourage you to plan ahead to ensure continuity in your medication. Thank you for your understanding and cooperation.

Medical Records Requests

To request copies of your medical records, please contact our office during regular business hours.

Please note that we are unable to process medical record requests during evenings, weekends, or holidays. We appreciate your patience and will fulfill your request as promptly as possible.

Cancellation, Late, & No-Show Policy

We value your scheduled appointment time and strive to ensure every patient receives timely care. Arriving late may impact other patients who are on time for their appointments. If you're running behind, we may need to reschedule your or your child’s visit to accommodate everyone appropriately.
