We are dedicated to supporting the residents of Columbia, South Carolina, and the greater Midlands area through a variety of programs designed to provide patients with access to exceptional health care.

This practice serves all patients regardless of inability to pay.

Discounts for essential services are offered based on family size and income.

Patient Information | What to Know Before Your Visit

What to Bring to Your Appointment

  • Valid photo ID

  • Insurance card (if applicable)

  • Co-pay or payment for your visit

  • A list of current medications

  • Immunization records (especially for physicals)

  • Relevant documents from recent ER, urgent care, or specialist visits

Accepted Insurance & Payments

We accept:

  • Medicare

  • Medicaid

  • BlueCross BlueShield

  • Select private insurance plans

No insurance?

  • We offer transparent flat-rate pricing. A $125 deposit is required at check-in for self-pay patients.

Payment methods accepted:

  • Cash, Visa, MasterCard, Discover, American Express, HSA/FSA.

Patient Financial Policy

By receiving services at Bella Clinical Care, you agree to:

  • Pay co-pays and deductibles at the time of service

  • Be responsible for any charges not covered by your insurance

  • Allow Bella Clinical Care to bill your insurance directly

Labs and other external services may be billed separately.

After-Hours & Emergency Care

We do not offer 24/7 on-call services.

For urgent medical concerns outside of regular hours:

  • Visit your nearest urgent care or emergency room

  • In a medical emergency, call 911 immediately

Referrals & Authorizations

If your provider recommends a specialist, our team will help coordinate the referral.

Note: Some insurance plans may require prior authorization, which can take up to 3 business days.

Prescription Requests & Refills

Prescription refills for existing patients are processed during normal business hours.

  • No refills are issued on weekends, holidays, or after hours

  • Please call ahead to avoid gaps in medication

Medical Records Requests

To request your records, call our office during business hours.

Requests are not processed on weekends, evenings, or holidays.

Cancellations, Late Arrivals, & No-Shows

  • Please arrive on time for your scheduled appointment

  • Late arrivals may be rescheduled based on provider availability

  • No-call, no-show appointments will be charged a $25 fee

  • If you need to cancel, please call at least 24 hours in advance
